Outpatient
Ideally, subjects should register themselves. Registration staff are the experts at obtaining full, and accurate information. The easiest way to do this would be when you tell the subject that you want them to come in for a visit, they first must register. You can transfer them directly to the registration phone number, state to the registrar that this is a research subject, under the CRC grant. The phone number is 7-4929. This line is staffed 8 AM to 11 PM. If you are off site, which does not allow direct access to the registration system, please ask the subject if they are willing to dial directly to 667-4929.

Inpatient
Please use the following pre-admission form when requesting CRC admission. This will allow admitting to capture all of the required information before a participants comes to the research center.
